Required Parental Rights Form - Senate Bill 12

Texas Senate Bill 12 now requires that schools obtain parental consent before providing specific services to students. These services may include:

  • Health Services - Nurses now need parent permission for care like first aid, health checks, and screenings (unless it's an emergency).

  • Counseling and Support Services - Parent consent is required before students can participate in guidance lessons, goal-setting, or short-term support services.

  • Student Clubs - Parents must give written consent before students can participate in any school-sponsored clubs.

As a result of this legislation, we need the following form completed for each child attending Cooper ISD in order for us to provide those services. Without your consent we will not be able to provide these supports to your child--even if they have received them in the past.
